For barbers, hair-replacement specialists, patients, clinicians and billing teams who need a clear starting point.
Important: A code describes an item, condition or service; it does not guarantee payment. The diagnosing clinician selects diagnosis codes, the treating provider selects procedure codes, and each payer decides eligibility, authorization, network and documentation rules. This page uses FY 2026 codes for dates of service through September 30, 2026; verify the FY 2027 set for services on or after October 1, 2026.
2026 diagnosis and product-code directory
This directory focuses on codes that directly name hair loss, alopecia or a behavioral or congenital condition that causes it. Confirm every code against the official code set for the date of service.

The product code: HCPCS A9282
CMS’s HCPCS index lists A9282 as “wig, any type.” Payers may use different benefit language such as wig, cranial prosthesis, scalp prosthesis or hair prosthesis. Verify whether the plan recognizes the item, requires prior authorization, limits the diagnosis or supplier, and treats it as covered, supplemental or excluded.
Codes that may accompany diagnosis and treatment
These are not hair-unit fitting codes. They may appear when a licensed clinician evaluates, diagnoses or medically treats hair loss. They must describe the service actually performed.
- 99202–99205 and 99211–99215: New and established outpatient evaluation and management.
- 11102–11107: Skin or scalp biopsy family; selection depends on technique and additional specimens.
- 11900–11901: Intralesional injection into skin lesions.
- J3301 / J3304: Triamcinolone drug supply when applicable; units and formulation matter.
- 96900, 96910, 96912 and 96920–96922: Selected phototherapy or laser services when medically indicated.
- 15775–15776: Punch-graft hair-transplant procedures, often cosmetic or excluded.
Coverage is not the same across Medicare, VA and private plans
Do not assume a wig or hair unit is a standard Part B benefit. Confirm the current national and local policy before accepting assignment or promising reimbursement.
A plan may offer a supplemental wig benefit for chemotherapy-related hair loss. Benefits, limits, networks and authorization rules vary by plan.
Some enrolled Veterans with alopecia or chemotherapy-related hair loss may be eligible when there is medical need and a VA referral or consult.
Coverage is contract- and state-specific. Ask about approved diagnoses, allowable amount, frequency limit and supplier requirements.
Documentation checklist for a medical hair-replacement request
- Clinical diagnosis: The treating clinician documents the actual cause and pattern of hair loss and selects the most specific supported ICD-10-CM code.
- Prescription or written order: Use the payer’s preferred terminology and include the condition, item requested and expected duration of need when required.
- Medical-necessity narrative: Document treatment-related hair loss, disease, burns, scarring, functional impact or mental-health impact only when clinically true and relevant.
- Benefit verification: Record the representative, date, reference number, authorization requirement, network rule, limit, deductible and patient responsibility.
- Supplier eligibility: Confirm whether the payer requires an enrolled or accredited supplier, licensed professional, contract, NPI, taxonomy, surety bond or electronic claims enrollment.
- Delivery record: Keep the signed delivery confirmation, item description, invoice, fitting notes, warranty and any required proof of delivery.
BEFORE KSI BILLS A PAYER
Build the supplier side correctly.
John King’s barbering and hair-unit expertise is the service foundation. Insurance billing is a separate compliance operation that may require business credentialing, payer contracting, an NPI and taxonomy, accreditation or DMEPOS enrollment depending on the payer and benefit.
Discuss a hair-unit consultationFrequently asked questions
What is the general ICD-10-CM code for hair loss?
L65.9 is nonscarring hair loss, unspecified, but the diagnosing clinician should select the most specific code supported by the patient’s record.
What code is used for alopecia areata?
L63.9 is unspecified alopecia areata. More specific L63 codes apply when the documented pattern supports them.
What is the code for chemotherapy-related hair loss?
There is no single universal code for every case. The clinician may document anagen effluvium or another supported diagnosis and any relevant treatment relationship. The payer’s policy determines required coding.
Does A9282 guarantee insurance will pay?
No. A9282 identifies a wig but does not establish medical necessity, eligibility, authorization, supplier status or coverage.
Can a barber diagnose alopecia or choose a medical diagnosis code?
A barber or hair-replacement specialist should not diagnose a medical condition. The treating clinician documents the diagnosis and the code must match that documentation.
